Resumo: A 42-year-old male patient, social drinker and 28-year-old smoker, attended a dental consultation complaining of a painful tongue cleft that bled when cleaned with a tongue scraper for approximately 3 months. Clinical examination revealed a linear cleft on the right side of the dorsal tongue with invagination and an ulcerated area at its most anterior surface, non-painful to palpation, approximately 3cm in length. Topical corticosteroid was prescribed for fifteen days without clinical improvement. An incisional biopsy was performed in the anterior third of the dorsal tongue. Histopathological examination was conclusive for severe epithelial dysplasia. The patient was referred to a head and neck surgeon and is undergoing surgical planning. Since patients with severe epithelial dysplasia have a high probability of malignant transformation, identifying and knowing the degree of dysplasia is mandatory for the early diagnosis of possible cancerous lesions.
