Resumo: Human papillomavirus (HPV) is commonly associated with oropharyngeal cancer, with HPV16 and HPV18 being the most common strains. However, association with HPV33 is extremely rare. We present the case of a 57-year-old man who exhibited enlargement of the tonsils and soft palate on the left side. The affected area displayed an ulcerated surface along with white plaque formation. He identified the lesion approximately six months prior and has recently experienced a heightened sensation of pain, especially when swallowing. The patient underwent incisional biopsy, and microscopic examination revealed a malignant epithelial neoplasm, characterized by neoplastic cells exhibiting a basal phenotype, an infiltrative pattern of invasion without the presence of horny keratin pearls, and tumor cells often showing individual keratinization. Immunohistochemical staining for Ki67, P16, CK5/6, CK14, P63, and P40 was positive. HPV genotyping uncovered the existence of HPV33, a high-risk strain that is infrequently reported in this region.
