Resumo: Adenoid Cystic Carcinoma is a neoplasm in smaller salivary glands, with a high risk of metastasis after long years of treatment. Objective: Analyze the histopathological grading systems of Adenoid Cystic Carcinoma and the specific survival of the disease. Study Design: For this systematic review, articles were conducted in the electronic databases: PubMed, Web of Science, Scopus, Science Direct, Google Scholar, and 13 cohort articles were included. Results: Of the 13 articles included in the systematic review, only 6 met the eligibility criteria for entry into the meta-analysis, with a statistically significant association between tumors with a solid pattern and minor time of specific survival of the disease (p = 0.003; 95%CI 3.42 [1.50 - 7.78]; I2 67%). Regarding the quality assessment of the studies by Quality in Prognostic Studies (QUIPS), four studies were of high quality, five articles received moderate quality, and two articles received low quality. Conclusion: It was concluded that the presence of any solid component contributes to a worse prognosis and lower specific survival of the disease over the 10 years of follow-up. Perineural invasion and surgical and radiotherapy treatment modalities need to be evaluated for prognosis in future studies.
