Resumo: The aim of this study was to report the use of the operating microscope in evaluating a patient with localized juvenile spongiotic gingival hyperplasia (LJSGH). An 18-year-old female presented for evaluation due to gingival redness and bleeding. Clinical examination revealed gingival recession in the maxillary canines, along with edema and erythema on the buccal surface. Both canines were submitted to orthodontic traction. Gingival details were evaluated with magnification (operation microscope), enabling visualization of the irregular gingival surface, superficial reddish vascular dots, and exudation. Incisional biopsy confirmed the diagnosis of LJSGH. The management consisted of plaque control and prophylaxis every 4 months and topical corticosteroids during exacerbations. This was the first case of LJSGH evaluated by direct oral microscopy. Most of the gingival details such as surface irregularities, petechiae and exudation were not observed in conventional clinical examination. Operating microscope may be an option for better evaluation of superficial oral lesions.
