Resumo: A 38-year-old woman, a former smoker, presented with swelling in the lower third of the face for two years of duration. She reported a chewing discomfort and no painful symptoms. A computed tomography showed a multilocular hypodense area, in the left posterior region of the mandible. It displayed well-defined limits and an expansion of bone cortices. Under the hypothesis of ameloblastoma, an incisional biopsy was performed and showed biphasic cellular proliferation. It was characterized by a dense and diffuse population of small, round cells, a hemagiopericytomatous pattern, and focal areas with well-differentiated cartilage. Immunohistochemistry for S100, CD99, CD56, AE1/AE3, FLI-1, CD45 LCA, desmin and Ki-67 was performed, revealing positivity for S100, CD99, CD56, FLI-1 and Ki-67. The morphological and immunohistochemical aspects were consistent with mesenchymal chondrosarcoma. The patient was referred to a head and neck surgeon for appropriate treatment.
