Resumo: Syphilis is a sensitive condition to primary health care (PHC), a reemerging infection. The oral manifestations can be challenging due to their ''great imitator'' nature. We report three cases with clinical descriptions and serological tests. Case 1: a 19-year-old male with a one-month history of painful oral lesions. Case 2: a 3-year-old female presenting with multiple oral lesions one month ago. Case 3: a 62-year-old male with a rapidly growing of painful oral lesions. Clinically, all cases showed the typical characteristics of syphilis. However, recent medical history revealed previous diagnoses and treatments for other oral diseases. Treponemal rapid test performed was reactive in cases 2-3 and non-reactive in case 1 on initial screening. Subsequently, treponemal and nontreponemal tests confirmed the diagnosis of acquired syphilis in case 1. All cases were treated for syphilis in PHC, interrupting the transmission chain and avoiding referral to other levels of care.
