Resumo: A 10-year-old male patient presented with progressive, asymptomatic mandibular enlargement over a 12-month period. Computed tomography (CT) imaging revealed a hypodense unilocular lesion with involvement of an unerupted tooth. An incisional biopsy was performed, and histological analysis showed an epithelial lining characterized by cuboidal and columnar cells resembling those found in the internal epithelium of the enamel organ, along with loose connective tissue with a fibromyxoid appearance. The diagnosis was established as a primordial odontogenic tumor. The primordial odontogenic tumor emerged as a novel entity documented by Mosqueda-Taylor et al. in 2014, and subsequently incorporated into the World Health Organizations histological classification under the category of mixed odontogenic tumors in 2017. The literature describes approximately 24 cases, predominantly affecting posterior mandible in young patients, without reports of recurrence. In our case, subsequent enucleation of the lesion was conducted, and the patient is currently under follow-up, exhibiting no signs of recurrence.
