Resumo: Polymorphous adenocarcinoma (PAC) is a malignant tumor frequent in minor salivary glands, characterized by cytology uniformity, architectural diversity and usually excelent prognosis. PACs harbor recurrent PRKD1 E710D hotspot mutations in >70% of cases. A 52-year-old black woman with a painless submucosal nodule in the superior lip, without ulceration and assymtomatic. Personal, familiar and medical history did not include relevant information. Due to the clinical hypotheses of canalicular adenoma or pleomorphic adenoma was performed a incisional biopsy and histological features revealed the typical characteristics of a PAC of minor salivary glands, with predominance of uniform neoplastic cells, frequently arranged in a trabecular pattern and an infiltrative area to the connective tissue. Neoplastic cells were diffuselly positive for S100 protein. The patient was referred to an oncology unit for surgical treatment and adjuvant therapy. Currently, the patient has been in follow-up for 6 months without recurrence.
