Resumo: Neurofibroma, a benign peripheral nerve neoplasm that can arise as a solitary tumor or be a component of neurofibromatosis, is uncommon in the oral cavity. A 28-year-old man was referred for evaluation of a slow-growing swelling in the tongue, that evolved over an approximately 14-year period. Intraoral examination revealed a firm, irregular, pinkish to yellowish nodular mass in the tongue, extending from ventral surface to dorsum, measuring approximately 5 cm. Considering the diagnostic hypothesis of mesenchymal neoplasm, an incisional biopsy was performed. Histopathologic analysis disclosed proliferation of spindle-shaped cells interspersed with collagen bundles and variable amounts of myxoid matrix. Immunohistochemical analysis revealed positivity for S-100 protein in most spindle-shaped cells and low proliferation index (Ki-67 < 5%). These findings were consistent with the diagnosis of neurofibroma. Treatment consisted of surgical excision and the previous histopathologic diagnosis was confirmed. No signs of recurrence were detected after 4 months of clinical follow-up.
