Resumo: Acanthosis nigricans is characterized by papillation of the mucosa and skin, hyperkeratosis, and hyperpigmentation. We describe a 46-year-old female patient with poor differentiated gastric adenocarcinoma with multiple metastasis showed multifocal oral lesions characterized by hyperpigmented velvety/warty plaques of pink coloration spongy to palpation throughout the extent of the upper and lower lips, hypochromic velvety/warty plaques on bilateral buccal mucosa and gum with oedema sensitive to touch with a velvety/warty appearance. In association with the lesions in the mouth, the patient presented dark lesions with a rough surface in the armpits, back, abdomen, and face reaching the diagnosis of Leser-Trčlat sign. Associating the type of tumor and the alterations observed in the physical examination, the diagnosis was acanthosis nigricans as a paraneoplastic syndrome. Patients presenting with these dermatoses should be suspected to harbor an occult malignancy, or have persistence of a known malignancy, until proven otherwise.
