Título: SURVIVAL AND PROGNOSTIC PARAMETERS IN HEAD AND NECK CANCER: A RETROSPECTIVE COHORT STUDY OF 3,052 PATIENTS IN BRAZIL

Resumo: Objective: To evaluate whether sociodemographic, clinical characteristics and lifestyle habits at diagnosis of head and neck cancer (HNC) are associated with the overall survival (OS) and mortality risk across different HNC anatomical sites.
Study Design: A retrospective cohort (2011-2021) at an oncological center in São Paulo, Brazil, including 3,052 HNC patients. Survival analyses utilized the Kaplan-Meier method and Cox proportional hazard ratios and their corresponding 95% confidence intervals, including variables associated with survival based on univariate analysis.
Results: Five-year OS was 31.4% for all HNC sites and 37.0% for larynx, 35.8% for oral cavity, 26.6% for oropharynx, and 14.8% for hypopharynx. HPV-positive patients demonstrated 59.3% five-year OS, compared to HPV-negative patients with a five-year OS of 35.4%. Multivariable analysis identified older age (≥70 years) and advanced tumor stage (IV) as independent adverse prognostic factors for OS in the oral cavity, oropharynx, and larynx sites. Alcohol drinking at diagnosis was also identified as an unfavorable prognostic factor in the oropharynx and larynx sites.
Conclusion: The OS rate for HNC remains low at a tertiary cancer center in São Paulo, Brazil. Older age, advanced tumor stage, and alcohol drinking at diagnosis are independent adverse prognostic factors, especially in oropharynx and larynx sites.
