Título: EPSTEIN-BARR VIRUS-POSITIVE MUCOCUTANEOUS ULCER: CASE REPORT

Resumo: Epstein-Barr virus (EBV)-positive mucocutaneous ulcer (EBVMCU) is a proliferative B-cell disorder with isolated lesions on the mucosal tissues. Associated with immunosuppression, such as use of methotrexate. The prognosis is favorable and screening for large B-cell lymphoma requests a multidisciplinary approach. A 69-year-old female patient treating Clippers Syndrome with methotrexate complaining pain in the oral cavity. Examination showed multiple necrotic ulcers with flat edges and erythematous halo in different regions of keratinized mucosa. Incisional biopsies were performed for histopathological analysis, bacterial and fungal cultures and viral PCR. Cultures were not conclusive. The PCR was positive for EBV. Therefore, a blood PCR was requested which was also positive for EBV. Immunohistochemical study and in situ hybridization for atypical lymphoid cells was positive for EBV. The diagnostic conclusion was EBVMCU, without lymphoma signals until now. Methotrexate was discontinued and there has been observed improvement of oral lesions and pain, but no complete resolution.
