Resumo: Odontogenic myxoma is a benign neoplasm originating from the ectomesenchyme with locally aggressive characteristics. This work aims to report the case of an 81-year-old woman, referred by a prosthetist due to bulging in the bottom region of the right maxillary groove. On physical examination, it was possible to observe effacement of the nasolabial fold on the right side and occlusion of the nostril with loss of air perfusion on the same side, due to an expansive process. The bottom of the maxillary sulcus was erased and there was expansion in the palatal region. The CT showed a well-defined osteolytic expansive process, affecting the entire maxilla, extending to the orbit, pterygoid region and occluding the nasal fossa. An incisional biopsy was performed with a diagnosis of odontogenic myxoma.Treatment consisted of excision of the tumor intraorally and the patient is being followed up for 2 years with no signs of recurrence.
