Resumo: Mucormycosis, an infrequent yet often lethal fungal infection, can occur particularly in immunocompromised individuals. It demonstrates swift progression, necessitating a more invasive surgical approach with heightened recurrence rates and postoperative complications. This case report details the presentation of aggressive rhino-orbito-cerebral mucormycosis in a 23-year-old woman with uncontrolled type 1 diabetes mellitus following the extraction of her lower left third molar. The patient underwent invasive surgical intervention coupled with antifungal therapy comprising amphotericin B, posaconazole, and anidulafungin. Subsequently, she developed acute osteomyelitis after one year, requiring bifrontal craniectomy and antibiotic therapy with amoxicillin and meropenem. Following multiple surgeries, the patient reported brief, shock-like pain in the left V2 region, elicited by tactile stimuli and functional activities, with radiation to teeth 23, 44, and 45. Clinical examinations revealed neuropathy in the V1/V2 region on the left side, suggestive of post-traumatic trigeminal neuropathic pain. Treatment involved carbamazepine at 400mg/day, with full control of the pain condition.
