Resumo: A 24-year-old female patient, with a history of a metastatic undifferentiated soft-tissue sarcoma in the left mandible experienced restricted mouth opening following chemoradiotherapy. Clinical examination revealed by a 12mm mouth opening, palpable hardening and pain in the left masseter muscle bundle, hyperalgesia in V3, and increased discomfort during mouth opening movements wich characterized mandibular trismus. Surgical removal of fibrosis in the left masseteric region was performed, followed by botulinum toxin type A (BTA) treatment. BTA was administered extraorally with 90 IU in the right masseter muscle, 45 IU in the left masseter muscle, and intraorally with 30 IU at two points in the fibrotic area. Additionally, the patient underwent local physiotherapy and heat therapy to facilitate mouth opening. On the 15th postoperative day, mouth opening measured 13 mm, which increased to 20 mm after 4 months, accompanied by mild discomfort after prolonged chewing.
