Título: Radiometric Analysis of Risk Structures in Head and Neck Cancer Patients Undergoing Intensity-Modulated Radiotherapy

Resumo: The treatment of the head and neck squamous cell carcinoma (SCC) often involves radiotherapy, employed both definitively in non-surgical scenarios and adjuvantly post-surgery. Intensity modulated radiotherapy (IMRT) is the preferred technique. This method achieves highly conformal dose distribution, facilitating the application of high doses to the target volume. However, the radiation doses delivered to areas surrounding of the tumor, such as the parotid glands and mandible, remain largely unknown. This study aimed to quantify these radiation doses in 28 SCC patients undergoing IMRT, comprising 15 patients with oropharyngeal SCC and 13 with oral SCC. The mandible in oropharyngeal SCC patients received higher average secondary radiation doses during treatment (35.57 Gy); the parotid glands in this group received average doses ranging from 24.82 Gy to 25.56 Gy. In oral SCC cases, the highest average mandibular doses were observed in patients with tumors located on the floor of the mouth (43.37 Gy), followed by those with tongue (35.50 Gy), retromolar triangle (36.22 Gy), buccal mucosa (30.63 Gy), and palate tumors (29.69 Gy). In summary, our findings highlight that the mandible receives the highest average doses of secondary radiation during IMRT for both oropharyngeal and oral cavity SCC.
