Resumo: Palisaded encapsulated neuroma (PEN) is a benign neural neoplasia of uncertain etiology, which usually presents as a slow-growing, asymptomatic, well circumscribed submucosal lesion. We report a case of a 21-year-old female melanoderm patient complaining about a bleeding lesion on the tongue. Clinically, a delimited nodule was observed, with an ulcerated surface on the posterior tongue dorsum. Clinical hypothesis was pyogenic granuloma and an excisional biopsy was performed. Histopathological analysis showed spindle cells in fascicles with nuclei exhibiting a certain parallel orientation. Cells demonstrated immunohistochemical positivity for S-100 protein. No recurrence of the lesion was observed in 18 months of follow-up. Although uncommon, PEN should be considered as a hypothesis in nodular lesions on the tongue dorsum.
