Resumo: This study aimed to investigate the prevalence of histopathological diagnoses of conditions affecting the gingival tissues, this is a retrospective longitudinal observational study of histopathological reports of gingival biopsies from a universe of 28,977 cases and a sample size of 1,695 gingival biopsies performed between 2001 and 2021. Sociodemographic data such as sex, age, intraoral location and histopathological diagnosis were obtained from the records. The most prevalent manifestations were inflammatory gingival hyperplasia (23.02%), peripheral ossifying fibroma (12.39%), pyogenic granuloma (9.68%), hyperkeratosis (8.32%), chronic inflammatory process (8. 26%), peripheral giant cell lesion (6.08%) and others with a prevalence of less than 5%, such as squamous cell carcinoma with a prevalence of (1.18%). Rarer manifestations included lipoma, sialoadenitis and ameloblastoma and amounted to (0.06%). Malignancies such as osteosarcoma and rhabdomyosarcoma represented (0.12%), and sarcomatoid, adenoid cystic and basaloid squamous carcinomas amounted to (0.06%). In conclusion, gingival biopsy is essential to confirm the diagnosis and plan treatment, considering the variety of diagnoses, some of which potentially life threatening.
