Resumo: An otherwise healthy 12-year-old girl was referred for evaluation upon an incidental radiographic finding during orthodontic treatment planning. Imaging exams revealed a 20 mm well-defined unilocular radiolucency containing a delicate peripheral radiopaque rim, closely related to the unformed crown of the lower third molar germ. An excisional biopsy was performed under local anesthesia. Grossly, a 10 mm fragment of a tooth germ was accompanied by a 10 mm round whitish mass of hard consistency and cystic eggshell appearance on cut surface. Microscopic evaluation revealed a cyst lined by ameloblastic epithelium, surrounded by an outer layer of mineralized dentin and primitive dental papilla-like ectomesenchyme. The tooth germ revealed features consistent with an aberrant odontogenic process. The final diagnosis was primordial cystic odontoma (PCO) associated with an aborted third molar. We report an additional illustrative case of PCO, an ill-defined entity that should not be misinterpreted as other odontogenic lesions.
