Resumo: Ameloblastoma is an odontogenic tumor that requires invasive treatment to reduce
the recurrence. In aggressive cases, immediate reconstruction with a vascularized free
fibula flap is indicated. This study reports a case of a 15-year-old male patient
presenting facial asymmetry, swelling in the left retromolar region and radiographically
had a unilocular radiolucent area associated with tooth 38. The anatomopathological
diagnosis of incisional biopsy was ameloblastoma. 14 months after diagnosis, it was
possible to observe extensive tumor growth, with cortical invasion along the entire
length of the left hemimandible. A hemimandibulectomy was performed with
reconstruction using vascularized free fibula flap. There was no recurrence in 8 months
of follow-up. Function and aesthetic remain satisfactory.
