Resumo: Sialadenoma papilliferum (SP) is a rare polypoid benign neoplasm of the salivary glands characterized by both exophytic and inward papillary proliferation of mucosal and salivary ductal epithelium, which mainly occurs in the hard palate and buccal mucosa of older males. A 58-year-old female patient presented an asymptomatic, 2 cm normal-colored papillary nodule of 5 months duration in the upper lip. The lesion was surgically excised, exhibiting a gross appearance of a 0.8 cm soft brownish fragment of irregular surface. Microscopic analysis showed an exophytic surface squamous proliferation directly contiguous with submucosal salivary gland ductal epithelium showing endophytic papillary infoldings and cystic-like spaces surrounded by plasma cell infiltrate. No recurrence was observed after 6 months of follow-up. SP may rarely affect the upper lip of older women. Clinicians should consider SP during the clinical evaluation of papillary lesions of the oral cavity.
