Resumo: Calcifying aponeurotic fibroma (CAF) is a rare benign tumor of the distal extremities of children and adolescent boys, with potential for local recurrence. Only two cases of oral CAF have been reported in the literature. An 89-year-old man presented an asymptomatic, normal-colored, and smooth-surfaced nodule in the buccal mucosa lasting two years. After conservative surgical removal, the gross examination revealed a 1.3 cm round fibrous tissue with a whitish cut surface and a lobulated gritty appearance. Microscopic evaluation showed a fibrous proliferation of plump spindle cells surrounding calcified fibrous nodules with hyalinized and chondroid areas. Tumor cells were positive for vimentin and negative for CD163 and CD34. Chondroid areas showed positivity to S-100 and D2-40. The final diagnosis was CAF. The patient is well, with no recurrence after 12 months of follow-up. This is an exceedingly rare case of CAF affecting the buccal mucosa of an older patient.
