Resumo: Ectomesenchymal chondromyxoid tumor (ECT) is a rare mesenchymal neoplasm of indolent clinical course that usually affects the anterior dorsal tongue of young adults. A 67-year-old man presented a slow-growing, painless, dome-shaped submucosal nodule in the anterior dorsal tongue of three years duration. Under the clinical hypothesis of granular cell tumor, an excisional biopsy was performed under local anesthesia. Grossly, a 1.2 cm well-circumscribed fibrous tissue fragment was observed, exhibiting a myxoid, slight lobular cut surface with a central blackish round spot. Microscopic evaluation showed an unencapsulated proliferation of spindle and polygonal bland cells in a multilobulated chondromyxoid stroma. The tumor cells showed eosinophilic cytoplasm, round nuclei, and positivity for GFAP, S-100, and CD57, with Ki-67 index labeling of 5%. No recurrence was observed after 6 months of follow-up. Older patients may be affected by ECT exhibiting the typical clinical appearance of an anterior dorsal tongue nodule.
