Resumo: Basaloid squamous cell carcinoma (BSCC) is an aggressive variant of squamous cell carcinoma primarily seen in the upper aerodigestive tract, including the oral cavity, larynx, and esophagus. A 53-year-old Caucasian male complaining of a painful whitish lesion on the base of the right tongue measuring 1.5 cm with an evolution time of 3 months. The patient presented enlarged right submandibular lymph nodes, dysphagia and otalgia. After biopsy, microscopic examination revealed overlying dysplastic epithelium exhibiting cellular and nuclear pleomorphism. Within the connective tissue, the tumor islands and lobules had basaloid cells with hyperchromatic nuclei, scanty cytoplasm and aberrant mitosis. Comedo necrosis was observed in the center of some lobules. The Ki-67 marker, detected by immunohistochemical reaction, was positive in approximately 70% of tumor cells. The final diagnosis of BSCC was obtained. The patient was referred to the oncology center for multidisciplinary treatment.
