Resumo: Recurrence of ameloblastoma in the temporal region is considered rare and associated with miss treatment at first approach. This study aims to report a case of recurrence of mandibular ameloblastoma first treated with surgical resection and replacement for temporomandibular joint (TMJ) alloplastic prosthesis, which failed four years later. The patient was male, 40 years old, and searched our service to treat the prosthesis exposure at the right preauricular region of his face. At the physical exam and with images of computerized tomography we could observe that he also presented a soft mass at the right temporal area, suggesting recurrence of primary tumour. We performed surgical removal of the TMJ prosthesis, associated with the resection of the temporal tumour, and used a microvascular free flap of the fibula to rehabilitate his mandibular bone. A histopathological exam confirmed the diagnosis of ameloblastoma. The patient is stable in 2 years follow-up.
