Resumo: Leiomyomas are benign tumors with smooth muscle differentiation, and they are rare in the head and neck region, with vascular differentiation being the most common type. This case report describes a 50-year-old female manifesting a painless nodule on the lower lip mucosa for approximately 7 months. Upon physical examination, the mucosal nodule was found to be indurated, well-defined, and displaying normal mucosal hue. An excisional biopsy was performed with a clinical suspicion of pleomorphic adenoma. Microscopically, a well-circumscribed encapsulated proliferation of spindle cells was observed, arranged in fascicles and/or surrounding blood vessel walls. Immunohistochemical analysis demonstrated strong cytoplasmatic positivity for smooth muscle actin (SMA) corroborating the diagnosis of vascular leiomyoma. The patient has since undergone a follow-up period of 6 months, exhibiting no evidence of disease recurrence.
