Título: MALT LYMPHOMA WITH EXTENSIVE PLASMACYTIC DIFFERENTIATION AFFECTING THE TONGUE: A CASE REPORT AND LITERATURE REVIEW

Resumo: A 47-year-old male presented with painful ulcers on the tongue six months ago. Intraoral examination revealed two ulcerated lesions on the dorsum of the tongue. Clinical differential diagnosis included syphilis, paracoccidioidomycosis, leishmaniasis and histoplasmosis. VDRL, FTA-Abs and HIV tests were all negative. Incisional biopsy showed a diffuse proliferation of plasma cells, without cellular pleomorphism, admixed with scarce lymphocytic foci, which infiltrated diffusely skeletal muscle bundles. By immunohistochemistry, CD7a, CD138, MUM1/RF4, IgG, and Bcl-2 were positive; while CD10, CD23, CD30, CD56, Bcl-6, cyclin D1 and IgG4 were negative. Kappa than lambda light chain highlighted a higher number of cells. Ki-67 was 4%. EBER1/2 was negative. Plasma cell mucositis was initially suggested; however, the clinical evolution favored MALT lymphoma with florid plasmacytic differentiation. To date, 7 cases of MALT Lymphoma affecting the tongue have been reported, which should be considered in the differential diagnosis of tongue ulcerations.
