Resumo: This case report describes a 58-year-old female patient with sensitivity in the fundus of the mandibular vestibule for 4 months. Oroscopy revealed swelling with a flaccid consistency and a normal-colored surface in the region of teeth 41 to 43, compatible with a radicular cyst. The panoramic radiograph showed a unilocular radiolucent area in the apical region of teeth 31/41/42/43, root resorption and destruction of the cortical bone. After incisional biopsy and surgical enucleation, the histology revealed a cavity lined by stratified, non-keratinized, thin squamous epithelium, with loosely arranged cuboidal and suprabasal basal cells. Numerous ghost cells, basophilic or limited by multinucleated cells. There was odontogenic epithelium in the capsule, forming cords and islets of cells resembling ameloblasts. The fibrous capsule was cellularized and vascularized, with calcified material, moderate mononuclear inflammatory infiltrate and cholesterol crystals. The diagnosis was a calcifying odontogenic cyst and the patient is undergoing clinical and imaging follow-up.
