Resumo: Two female patients, aged 7 and 22, sought the Oral Diagnosis Center with unresolved gingival lesions following dental treatment. Both patients exhibited reddish enlargements of the attached buccal gingiva of the maxillary incisors. The clinical characteristics prompted the diagnosis of Juvenile Spongiotic Gingival Hyperplasia and they were instructed regarding oral hygiene and use of chlorhexidine mouthwash for two-weeks and scheduled for surgical excision. The histopathological analysis showed acanthotic stratified squamous epithelium with abundant spongiosis and exocytosis, and intense lymphoplasmacytic inflammatory infiltrate at the underlying lamina propria, confirming the clinical hypothesis. Juvenile Spongiotic Gingival Hyperplasia is an uncommon gingival lesion less known by periodontistis and paediatric dentists, which is not related to dental biofilm and usually observed in young patients. Recently was described as an odontogenic reactive disorder of the gingiva. The aesthetic impact of both the lesion itself and the surgical excision should be considered in the treatment decision.
