Resumo: A 62-year-old white man with a lesion on his soft palate was examined. According to the patient, the lesions were present for the last 2 years and progressively enlarged. The patients medical history was negative for previous illnesses. Intraoral examination revealed the presence of a well-defined asymptomatic red-violet nodular mass in the middle of the palate, measured 3x3.5cm in diameter. The histopathological analysis evidenced non-caseating epithelioid cell granulomas, observed in typical cases of sarcoidosis. The systemic organ involvement was negative, as well as the radiograph of the maxillofacial region did not show any abnormality. The patient was referred to a rheumatologist who prescribed 5mg prednisolone, with complete remission of the oral lesion in 6 weeks after the onset. The patient remains in follow-up, without signs of recurrence. Therefore, it is worth highlighting, that oral lesions may be the first or the only sign of sarcoidosis in an otherwise healthy patient.
