Título: ORAL HISTOPLASMOSIS MIMICKING TRAUMATIC LESION INDUCED BY REMOVABLE DENTAL PROSTHESIS

Resumo: A 77-year-old male was referred for evaluation of pain related to removable dental prosthesis that persisted for 40 days. The patient reported being previously treated by a private dentist, who made a diagnosis of traumatic ulcer and suggested the suspension of the prosthesis. It was also prescribed topical corticosteroids without significant effects. The patient was HIV negative. Intraoral examination revealed a purplish swelling with ulcerated areas located on the lingual gums of the teeth 43, 44 and 45, with provoked bleeding and pain. Imaging exams showed no bone alterations. The differential diagnosis included granulomatous infection and malignant neoplasm. An incisional biopsy was performed and histopathological examination discarded malignancy. Silver staining showed numerous and rounded structures suggestive of HIstoplasma capsulatum. The patient was treated with systemic itraconazole. In a 8-month follow-up the patient was asymptomatic and the lesion had completely regressed.
