Resumo: Plasma cells gingivitis (PCG) is a non-neoplastic oral disorder caused by a hypersensitivity reaction to an allergen such as chewing gum, toothpaste or specific foods.A 44-years-old woman consults for a gingival erosion with 2-month progression that extends in anterior maxillary attached gingiva and in minor proportion in mandibular attached gingiva. It produces a burning sensation and pain but with no bleeding. The clinical exam presents a homogeneous extended erythema with a negative Nikolsky sign. The all-laboratories results are normal, including VDRL and Elisa-test. Food allergy test was negative. A histopathological study showed presence of inflammatory lymphocytic infiltrate with predominating plasma cells. CD138, kappa and lambda immunohistochemistry were positive in equal proportion indicating polyclonally, therefore the diagnosis was PCG. Treatment included the use of lauryl-sulfate free toothpaste and application of topical corticoids for 21 days two times a day. At the control session, the gingival lesion and symptoms are total remission.
