Resumo: A 32-year-old woman, with no relevant medical history, presented due to a radiographic finding of an osteolytic lesion associated with the root of tooth 1.3, which had been endodontically treated. The lesion appeared rounded, well-defined, and asymptomatic. Cone-beam computed tomography revealed slight expansion of the vestibular table measuring 19.7 mm coronally-apical and 10.5 mm vestibulo-palatally. An excisional biopsy was performed, revealing loose connective tissue with abundant fibroblasts displaying prominent, spindle-shaped nuclei. Within this tissue, there was an irregular fibrous stroma associated with numerous trabeculae of irregularly shaped small to medium-sized lamellar bone tissue, some of which appeared cement-like, containing numerous osteocytes but lacking osteoblastic rimming, consistent with a diagnosis of cemento-ossifying fibroma. Currently, the patient is undergoing clinical and radiographic follow-up every 6 months, showing satisfactory healing without pathological changes.
