Título: RETROSPECTIVE ANALYSIS OF CONSERVATIVE TREATMENT OF UNICYSTIC AMELOBLASTOMA

Resumo: Aims: The aim of this study is to describe a retrospective series of cases of unicystic ameloblastoma treated with conservative surgery and with a long follow-up.
Material and methods: Over a 24-year period, clinicopathological and therapeutic data of unicystic ameloblastoma treated with conservative surgery were retrospectively reviewed. Unicystic ameloblastoma cases were classified based on three criteria: 1) clinical findings during biopsy: presence of a cystic lesion or positive aspiration puncture; 2) 3D imaging with contrast: well-defined unicystic lesion; 3) histopathological examination consistent with ameloblastoma. Marsupialization was performed as an initial surgical procedure following incisional biopsy. After marsupialization and a period of decompression, the lesion was enucleated, followed by cryotherapy or peripheral ostectomy.
Results: This study included 25 cases with a mean age of 18.6 years (range 4-45 years). All cases underwent conservative surgery, with 21 cases initially undergoing marsupialization before definitive surgery. The mean follow-up period was 85 months (range 7-276 months), with an average marsupialization duration of 11 months. Only one case experienced recurrence after 7.8 months following marsupialization and peripheral ostectomy.
Conclusion: Conservative treatment for unicystic ameloblastoma appears to be a reliable option with low rates of recurrence and minimal morbidity, particularly in younger patients.
