Título: PERIPHERAL ODONTOGENIC KERATOCYST: CASE REPORT

Resumo: Odontogenic keratocyst (OKC) is a frequent benign but aggressive intraosseous lesion of odontogenic origin that arises from dental lamina remnants or basal cells of the overlying epithelium. Peripheral OKC is an extremely rare extra-skeletal variant. Its main differential diagnosis is gingival cysts and mucoceles. We present the case of a 17-year-old female patient who presented with a purple nodular lesion located in the anterior sulcus. Radiographic images revealed no evidence of bone involvement. During the excisional biopsy, the surgeon confirmed intact cortical bone. Microscopic analysis revealed typical histological features of a keratocyst. The diagnosis of peripheral OKC was established through radiological and histological evaluation. Due to the patients history of multiple intraosseous keratocysts and the current peripheral keratocyst, screening for Gorlin syndrome was conducted but not confirmed, as other signs of the syndrome were absent.
