Resumo: Solitary fibrous tumor (SFT) is a rare mesenchymal neoplasm, whose biological behavior is not yet fully understood. We report the case of a 59-year-old woman, presenting a nodular lesion, measuring 3.0 cm its largest diameter, with a smooth surface, but discreetly lobulated and normocolored, located in the oral mucosa, with a previous report of TFS from an incisional biopsy. An excisional biopsy was performed, which revealed a proliferation of isomorphic spindle cells, sometimes diffuse, in short, intertwined fascicles, and eventually forming narrow, branched vascular channels, resembling a "deer antler". The tumor cells showed immunohistochemical positivity for CD34 and STAT6, confirming the diagnosis of TFS. The differential diagnosis of TFS can be quite challenging due to overlapping histopathological findings between this tumor and other benign and malignant spindle cell neoplasms of the mandible. Therefore, a discussion of the clinicopathological features and diagnostic criteria of TFS is also provided in this work.
