Resumo: The association of oral erythematous lesions and TSH levels is a rare event. However, along with conventional thyroid management, different oral manifestation and laboratory test must be considered. A 52-year-old female with past medical history relevant for 22 years of hypothyroidism and recurrent anemia, treated with 100 ug levothyroxine, has been complaining for 3 months due to burning in a various areas of her mouth, mainly on the tip of the tongue. On clinical examination, multiple erythematous lesions were mainly seen on the inferior labial mucosa and bilateral belly of the tongue. Laboratories THS 15.5 uIU/mL, FT 4 .78 ng/dL, anti-parietal cell antibodies 130.44 U, B12 Vitamin <148.0 pg/mL and homocysteine 20.5 umol/L. A diagnosis of thyrogastric autoimmune syndrome was established. Erythematous lesions in patients with uncontrolled hypothyroidism could indicate other autoimmune diseases and it is necessary to confirm with laboratory test.
