Resumo: A 78-year-old man, black, with hypertension, was referred to our service for evaluation of a bleeding lesion in the palate. The patient was diagnosed with multiple myeloma five-years ago, and currently, remains under onco-hematological treatment with Melphalan 2mg. One day before the consultation, he observed a blood blister on the palate after brushing his teeth. Intraoral examination revealed an ulcer in the soft palate, measuring about 2cm with blood clot formation and hemorrhagic areas. Laboratory tests were requested and showed intense thrombocytopenia. These findings led to the diagnosis of an oral manifestation associated with thrombocytopenia related to activation of multiple myeloma and Melphalan treatment. The patient was monitored by our team. Seven days after the initial assessment, the lesion displayed a significant decrease in size and signs of healing. After one month follow-up, there was complete healing of the mucosa with scar tissue formation.
