Resumo: A 50-year-old male patient with severe aplastic anemia undergoing treatment for the past 12 years at a local hematological service, was referred to the authors oral medicine service after the institutional dental care professional noticed on a panoramic radiograph a well-defined circumscribed radiolucent lesion in the periapical region of tooth 48. The patient had a history of several treatment interruptions due to distance and health accessibility issues. The patient did not present pain, edema, or swelling. Preoperative exams revealed intense thrombocytopenia (platelets = 26000/mm³ and MPV = 6.3 fL) requiring platelet transfusion to control potential bleeding. The 48 tooth extraction with cystic enucleation, and curettage were performed without complications. Histopathological diagnosis of radicular cyst led to clinical follow-up, mostly due to inferior alveolar nerve paresthesia, requiring 10 sections of low-power laser therapy. After 6 moths bone formation and total clinical recovery was achieved.
