Resumo: Malignant salivary gland tumors may present clinical features of benign neoplasms, mostly during its early stages. A 63 y.o. patient was referred to the oral medicine service, with a painless sessile nodule located on the buccal mucosa near the upper retroalveolar ridge, persisting for 3 weeks. Clinical examination revealed controlled hypertension and total edentulism in the upper arch. Preoperatory exams included CBC, INR and GL. Excisional biopsy was performed and the sample was sent for histopathological analysis based on a benign salivary gland tumor hypothesis. A low-grade mucoepidermoid carcinoma diagnosis followed by, immunohistochemical essay with the following results: Ki-67+ of 5-10%; P-63+; CerB-2/Her2-neu+; CK5/6+ in basal cells. The patient was referred to the local oncological service and due to the good outcome achieved is being monitored every 90 days period for the next 5 years.
