Resumo: Sexually transmitted infections (STIs), especially syphilis, are increasingly affecting young individuals and adolescents. The first case involves an 18-year-old male, caucasian, who presented with a main complaint of a verrucous white plaque extending bilaterally along the lingual margin, which was asymptomatic. The initial hypothesis was hairy leukoplakia, but histopathological examination (HE) and immunohistochemistry for Treponema pallidum were positive. The second case involves a female patient, caucasian, who complained of a persistent tongue lesion. She had habits of using water pipe, vape, and straw cigarettes. She presented with multiple ulcers on the dorsal tongue, erythema on the buccal mucosa, and lingual margin. Initially treated with anti-inflammatory mouth rinses, but due to the persistence of the lesions, excisional biopsy of one of the ulcers was performed. Given the high number of plasma cells observed in the HE, immunohistochemistry for Treponema pallidum was conducted, confirming the diagnosis.
