Título: ARCHITECTURAL AND IMMUNOPHENOTYPIC PROFILE OF ORAL GRAFT-VERSUS-HOST DISEASE: A SPATIAL SINGLE-CELL ANALISYS

Resumo: Objectives: To define the histopathological, architectural, and immunophenotypic profile of oral graft-versus-host disease (GVHD) using a multiplex immunofluorescence model and artificial intelligence (AI) algorithm for the identification of cellular phenotypes and tissue schematics. Study Design: Biopsies from GVHD patients were submitted to a multiplex immunofluorescence experiment containing immune and structural markers, such as: CD8, CD4, CD20, GZMB, FOXP3, HistoneH3Phosphone, EPCAM, Galectin-3, CD3e, HLA-a, HLA-dr, CD45, CD45RO, CD21, PD-1, PD-L1, CD14, MPO, CD68, GATA3, IDO1, Keratin 14, CD107a, CD31, CD34, CD141, ICOS, Keratin 8/18, Podoplanin, Collagen IV, SMA, CD79a, CD38, Bcl-2, Caveolin, IFGN, CD66, Vimentin, T-bet, CD56, CD1c AND PanCK. An AI model was used to assign cells according to the markers on a spatial-single cell approach, each cell coordinate was extracted to run a Delaunay triangulation neighborhood analysis. Results: Changes in structural cells consistent with fibrosis of the minor salivary gland were observed and patterns of cellular clusters with interactions between structural and immune cells were detected, mainly a correlation between a T-reg population and proliferative Fibroblast/Vasculature population demonstrating the diseases heterogeneity. Conclusion: This study provides valuable insights into the complex cellular dynamics underlying oral GVHD parenchyma replacement, offering potential avenues to improve early diagnostic tools and therapeutic strategies.
