Resumo: Rhabdomyoblastic differentiation is a rare form of presentation of dedifferentiated Liposarcomas. A 35 y.o. patient, smoker, was referred to the oral medicine service with complaints of oral left facial swelling with intraoral ulcers, in the lower left molar region. CT-scanning examination revealed an ill-defined hypodensity measuring 7cm of width in the affected area. Blood pressure examination revealed uncontrolled hypertension of 200/120Hgmm. Preoperatory exams included CBC, INR, BG, VDRL, EBV, HIV. With all preoperative tasks controlled, was performed an incisional biopsy followed by histopathological examination. The suggestive diagnosis of Spindle Cell Sarcoma led to immunohistochemical evaluation, with the following results: CDK4+; MDM2+; Myogenin+; SMA+; SOX10+; CD31+; CD34+; S100 focally +; PanCK-; Ki67 of 20-30%. A final diagnosis compatible with Dedifferentiated Liposarcoma with Rhabdomyoblastic Differentiation led the patient referral to head and neck surgery undergoing a wide margin himi-mandibulectomy and left neck dissection. The patient is under follow up to the date.
