Resumo: A man, 24 years old, was diagnosed with tongue squamous cell carcinoma, T3N2aM0 (S IVA). The anatomopathological analysis of the surgical specimen showed a moderately differentiated tumor, with disease extension to the floor of the mouth and 20.0mm infiltration depth, lymphovascular and perineural invasion; surgical margins were compromised, changing the staging to pT3N2b. Therefore, adjuvant chemotherapy was prescribed (high-dose cisplatin in 3 cycles, 21 days apart) associated with radiotherapy (54Gy in 30 fractions, until 04/28/2022). After 4 months, he presented complaints of pain in the left buttock region, irradiating to the ipsilateral lower limb. PET-CT showed a lytic lesion with a soft tissue component extending through the sacrum, iliac, and acetabulum on the left (SUVmax 6.5), confirming the bone metastasis bone. Zoledronic acid was prescribed in association with left sacrum/iliac and acetabulum anti-pain radiotherapy (30Gy in 10 applications, until 12/23/2022). Seven months after the bone progression diagnosis, the patient died.
