Título: DIFFERENTIAL DIAGNOSIS OF ORAL VASCULAR MALFORMATION ASSOCIATED WITH PHLEBOLITH: REPORT OF TWO CASES

Resumo: A 59-year-old man sought the Oral Diagnosis Center with a complaint of a 10-mm purplish fibrous nodule at the upper gingivobuccal sulcus. Differential diagnoses included vascular malformation, Kaposis sarcoma, and minor salivary gland tumor. Positive diascopy and a radiopaque area on periapical radiograph suggested vascular malformation with a phlebolith. The lesion was excised, and histopathological analysis revealed congested blood vessels and calcified structures, confirming the clinical hypothesis. Another 66-year-old male complained of transient painful swelling in the left hemiface with reddish-purple patches on the buccal and palatal mucosa. The differential diagnoses included vascular malformation, sialadenitis/sialolithiasis, and parotid neoplasm. Computed tomography revealed a hyperdense round structure, and normal saliva production, confirming the same diagnosis of vascular malformation with a phlebolith. Vascular malformations exhibit a wide spectrum of clinical presentations and should be considered in the diagnosis of purplish lesions. Occasionally, phleboliths may be observed due to blood stasis and thrombus mineralization.
