Resumo: Introduction: Follicular lymphoma is a type of non-Hodgkin lymphoma. The diagnosis is conventionally confirmed by lymph node biopsy that shows proliferation of B lymphoid cells. PET-CT and bone marrow biopsy is useful for staging. Presentation in the oral cavity is rare and the diagnosis use to be late. Objective: to report the recurrency of follicular lymphoma in the maxilla. Clinical case: male patient, 59 years old, undergoing chemotherapy treatment for relapse of grade III/stage IVB follicular lymphoma. He was referred by the hematology team due to complaints of gingival hyperplasia. The patient reported periodontal treatment and the use of mouthwash without improvement in the condition. Intraoral examination revealed generalized gingival hyperplasia in the maxilla, without bleeding and the absence of symptoms. The incisional biopsy revealed areas with lymphoid infiltrate consisting of small cells forming follicles. The immunohistochemistry confirmed the infiltration by the follicular lymphoma. The early identification allowed treatment adjustment.
