Resumo: Patient 66 years old, male, ex-smoker, diagnosed with prostate cancer with bone metastasis. The previous history of treatment for cancer revealed that the patient underwent chemotherapy, radiation therapy and made quarterly use of zoledronate for two years and ten months, which was discontinued after the appearance of necrosis in the jaw, noticed by the patient after 6 months of use of a new partially removable prosthesis. In the physical examination, a single lesion was observed with bone exposure and the presence of purulent secretion in the posterior region of the right jaw. A prescription of clindamycin was made and, subsequently, antimicrobial photodynamic therapy and the PENTO protocol, a medicinal combination of pentoxifylline and tocoferol was initiated. After 5 months of treatment with the combination of the PENTO protocol and aPDT therapy, total regression of bone exposure was observed.
