Resumo: An 11-year-old male patient, presented to our service due to an increase in volume in the anterior region of the hard palate and fundus of the vestibule, with a radiolucent image in the panoramic radiography. The patient had done a previous incisional biopsy, with diagnosis of central giant cell lesion. Endocrinology evaluation was requested, with concomitant follow up. An initial treatment with denosumab was instituted for calcification of the lesion. 13 applications of intralesional denosumab were performed, initially generating a good response, showing signs of calcification. However, after discontinuing treatment, the patient developed nausea and vomiting, with hypercalcemia being identified at the emergency room in our hospital, being treated with pamidronate and zoledronic acid. After the patients condition improved, he underwent 8 sessions of triamcinolone intralesional injections, due to new decalcification of the lesion, followed by surgical excision of the lesion.
