Resumo: A 49-year-old brown-skinned patient sought dental care for a persistent tongue lesion noticed three months prior, causing eating difficulties. The examination revealed a foul-smelling, erythematous, tumor-like lesion with ulceration and necrosis extending from the tongue to the oropharynx. An incisional biopsy with histopathological analysis revealed a proliferation of large cells with round nuclei, prominent nucleoli, and basophilic cytoplasm, suggesting lymphoproliferative neoplasia. The immunohistochemical analysis confirmed the diagnosis of diffuse large B-cell lymphoma, positive for BCL-2, CD20, CD45 LCA, c-Myc, Ki-67, and MUM 1. Referred to head and neck oncology followed by hematology-oncology, CT image examinations showed an isodense lesion in the tongue and oropharynx, extending to the neck, mediastinum, and abdomen, with no bone involvement. Lumbar puncture excluded CNS involvement. Chemotherapy led to lesion regression after the first session. Two months later, the patient continues treatment with a guarded prognosis due to the lesions extensive spread and delayed diagnosis.
