Resumo: A 52-year-old female was referred complaining of symptomatic gingival lesions for 2years, which did not regress after periodontal therapy. Extraoral examination andmedical history were noncontributory. On intraoral examination, reddish spots andirregular erythematous areas were observed involving the marginal and attachedgingiva of the maxilla and mandible. Periodontal examination showed normality,including absence of bleeding and suppuration. The blood tests were normal. Afterincisional biopsy, microscopy revealed an intense inflammatory infiltrate rich in plasmacells. HIV, VDRL and FTA-ABS tests were all negative. Given these findings, thediagnosis was plasma cell gingivitis, which showed improvement after local hygieneand corticosteroid therapy. The literature review shows that periodontal disease, lichenplanus, cicatricial pemphigoid (on clinical examination), as well as syphilis and plasmacell disorders (on microscopical examination) should be included in the differentialdiagnosis of plasma cell gingivitis.
