Título: PLASMA CELL GINGIVITIS: AN UNCOMMON ENTITY THAT COMMONLY MIMICS OTHER PATHOLOGICAL ENTITIES

Resumo: A 52-year-old female was referred complaining of symptomatic gingival lesions for 2
years, which did not regress after periodontal therapy. Extraoral examination and
medical history were noncontributory. On intraoral examination, reddish spots and
irregular erythematous areas were observed involving the marginal and attached
gingiva of the maxilla and mandible. Periodontal examination showed normality,
including absence of bleeding and suppuration. The blood tests were normal. After
incisional biopsy, microscopy revealed an intense inflammatory infiltrate rich in plasma
cells. HIV, VDRL and FTA-ABS tests were all negative. Given these findings, the
diagnosis was plasma cell gingivitis, which showed improvement after local hygiene
and corticosteroid therapy. The literature review shows that periodontal disease, lichen
planus, cicatricial pemphigoid (on clinical examination), as well as syphilis and plasma
cell disorders (on microscopical examination) should be included in the differential
diagnosis of plasma cell gingivitis.
